Sample Public Notice for Violation of the Water Testing Schedule

PUBLIC NOTICE

RE:  NOTICE OF A VIOLATION OF THE WATER-TESTING SCHEDULE

The (name of PWS) is required by Minnesota Rules, part 4720.1200, to test one water sample every three months for coliform bacteria.  This required sampling was not done for the months (month, month, year).  Previous testing shows that (name of PWS) water is safe to drink, that there is no contamination problem, and customers do not need to seek alternative water supplies.  The (name of PWS) regrets the mistake and will work to make sure that the testing schedule is observed in the future.

Customers with questions or concerns about bacteriological contamination, or any other water problem, should contact the local water supply.  Questions should be directed to (name, address, and phone number of PWS contact person) during the hours of (time).
